
So far, nearly all of the original Scrubs cast members have made an appearance in the show’s revival, especially after Neil Flynn returned as the Janitor in the season finale, “My Celebration.” However, a few familiar faces are still missing. Interestingly, there’s a strong fan theory suggesting that one of these missing characters has actually been at Sacred Heart Hospital this whole time – we just haven’t seen them since the revival began.
Everyone is eager to see Ken Jenkins return as Dr. Kelso in the new season of Scrubs, but he’s not the only original cast member who hasn’t appeared yet. One actor, who was in 49 episodes – practically a main character! – was noticeably absent from the first episodes of the revival. Fans are holding out hope he’ll be in Season 2, but are waiting to see if the show gets renewed first. Despite not being on screen, he’s still been busy with other projects.
Johnny Kastl’s Dr. Doug Murphy Could Easily Still Be Working at Sacred Heart in the ‘Scrubs Revival’

John ‘J.D.’ Dorian frequently called him ‘Nervous Guy,’ and Dr. Cox often did too, but Johnny Kastl was arguably the least skilled doctor on Scrubs. He made a lot of mistakes and, tragically, patients died because of his incompetence. It’s surprising he lasted as long as he did, especially since the show sometimes fired doctors who weren’t performing well. However, Scrubs was a comedy, and his lack of ability often led to humorous situations.
Doug’s career took an unexpected turn in Season 4 when he was temporarily reassigned to the hospital morgue. Surprisingly, this led to a positive change. He discovered a passion for pathology and became a highly skilled doctor analyzing patients’ deaths. The reason for his success, though grim, was that his past mistakes had given him a unique and unsettling ability to determine how people died. Working in the morgue allowed him to excel without the pressure of harming patients, ultimately transforming him from a nervous and unsure person into a confident and capable doctor.
Doug Kastl’s character continued to appear on Scrubs for eight seasons, even though it wasn’t necessarily logical. Since he worked in the hospital morgue—which is located in the basement—he often spent entire shifts down there. This same logic could explain why he hasn’t been seen in the Scrubs revival yet. He could simply be busy with his work in the morgue and hasn’t had a chance to appear on camera. In fact, the revival scene with JD and Carla cooling off in the morgue could easily be explained by Doug just not being on duty that day. It’s a perfectly reasonable explanation.
How Likely Are We To See Doug in ‘Scrubs Season 2’?

A revival of Scrubs is looking increasingly likely. Showrunner Aseem Batra explained to Deadline that bringing back the entire original cast alongside new actors proved too challenging for a single season. She confirmed plans to include more returning favorites in a potential Season 2, specifically mentioning John Kastl as someone she’d love to have back. She also revealed that Dr. Kelso will definitely return next year. The idea is to reintroduce characters like Doug seamlessly, as if they were present all along but simply working in the background – in Doug’s case, in the morgue.
Despite leaving acting after season eight of Scrubs, Ken Jenkins is now a lawyer. While he hasn’t said if he’d return for the revival, the show’s creator, Bill Lawrence, has indicated he’d like to include him. Encouragingly, Lawrence also successfully convinced Jenkins to come back if the show gets picked up for new episodes. This gives hope that Lawrence can persuade Kastl to make an appearance as well, even if it’s just a short one.
